To get the rebate, mail in your original UPC Bar Code, a printed copy of this rebate page, and a copy of your receipt. Also include your name, complete address, email and phone number. Your mailed-in rebate request must be postmarked at least 30 days from  the date of  your purchase.

  • Take a photo (via smartphone or digital camera) of readable receipt and UPC barcode from the NeilMed® product box. Save the photo to an easily accessible location on your PC or smartphone.
